China continues to refine the visa framework for medical travelers. 2026 brings three changes worth knowing about, plus a reminder of what hasn't changed.

What Changed

1. Faster S2 Processing in Most Markets

The standard processing window for short-term S2 medical visas remains 4 working days, but expedited service (24–48 hours) is now offered in more consulates than before. We've confirmed expedited service is currently available in: USA, UK, Germany, France, Italy, Spain, UAE, Saudi Arabia, Russia, Brazil, Mexico, South Africa.

2. Visa-Free Transit Compatibility

Several short visa-free transit categories (24h, 72h, 144h) have been refined and expanded. For pre-treatment scouting trips or short consultation visits, these can sometimes be used in lieu of an S2 — but they cannot be used for the actual treatment portion. 3. Digital Application Improvements

The COVA online application portal now supports more languages and accepts digital signatures for an expanded set of supporting documents. Hospital invitation letters can now be uploaded directly without paper-by-courier in most jurisdictions.

  2. Pre-feasibility review → senior physician opinion on case (3–5 business days)
  3. Treatment plan finalization → official quote and timeline
  4. Hospital invitation letter issued (1–2 business days after deposit)
  5. Visa application — standard 4 working days; expedited 24–48 hours where available
  6. Flight booking — only after visa approval is confirmed

Common Pitfalls (Still True)

  • Don't book non-refundable flights before visa approval
  • Don't apply on a tourist visa (L) and try to receive medical treatment — this is a misuse and can complicate future applications
  • Don't omit prior medical records in the application — partial documentation is the leading cause of S2 delays
  • Don't expect the hospital invitation letter to issue before deposit confirmation

For Multi-City Treatment Plans

If your treatment plan involves multiple hospitals across multiple Chinese cities (e.g., consultation in Shanghai → procedure in Hefei → follow-up in Beijing), the S2 visa is single-entity but allows internal travel without restriction.
