The Long Road
Ngozi and Emeka (names changed) are a Lagos couple in their late 30s. Five years of trying. Two failed cycles in Lagos, one in Dubai, one in Cape Town. Each time the issue had been framed differently: "egg quality" in Lagos, "sperm motility" in Dubai, "embryo selection" in Cape Town. None of the cycles had used preimplantation genetic testing for aneuploidy (PGT-A).
The Reframe
Our partner reproductive endocrinologist in Shanghai reviewed all four prior cycles in detail. Her conclusion was different from the previous diagnoses — not because they were wrong, but because they were partial. She identified:

The First Trip
The couple flew Lagos → Addis Ababa → Beijing → Shanghai (24 hours). Stayed 16 days for stimulation and retrieval.
Stimulation produced 12 oocytes. Eight fertilized via IMSI-ICSI. Five reached blastocyst. Three cleared PGT-A as euploid.
The Second Trip
Eight weeks later, after Ngozi's endometrium was prepared via a coordinated remote-monitoring protocol with her local OB-GYN, they flew back for a 5-day single euploid embryo transfer trip.
The Wait
Beta hCG day 9: 178. Day 11: 412. Day 13: 936. Their case manager Lin still has the screenshots Ngozi sent.
