What is Vedolizumab for UC?

An anti-α4β7 integrin antibody that selectively blocks gut-homing lymphocytes — providing gut-specific immunosuppression. Particularly effective for ulcerative colitis with excellent long-term safety profile (no systemic immunosuppression).

Who is a candidate?

• Moderate-severe UC despite mesalamine
• Failed or intolerant to anti-TNF
• UC with active extraintestinal manifestations limited (better for gut-only disease)
• Older patients or higher infection risk (preferred over anti-TNF)

Outcomes

Clinical response at 1 year: 50-60%. Steroid-free remission: 35-45%. Endoscopic healing: 30-40%. Loss of response: 10-15% over 5 years (lower than anti-TNF). Excellent safety profile in long-term studies.
