What is MSC Therapy for ALS?

Mesenchymal stem cells secrete neurotrophic factors (BDNF, GDNF, VEGF) that may slow motor neuron degeneration in amyotrophic lateral sclerosis. While not curative, early-phase trials suggest slowing of disease progression in some patients.

Who is a candidate?

• Definite or probable ALS (El Escorial criteria)
• Disease duration 6-36 months (best responders)
• FVC >50% predicted
• ALSFRS-R ≥30
• Realistic expectations — this is disease-modifying, not curative

Outcomes

Disease progression slowing observed in subset of patients in phase II trials (~30%). ALSFRS-R decline rate reduction in some studies. Safety profile favorable — no major adverse events. Important: Benefits are modest; inform patients this is not a cure but may extend function.
